There are many methodologies on the market today; your company may already be using one. For our customers that have not adopted a methodology yet, we bring ours. We call our methodology LM3, which stands for Lancet Models and Methodology Manual. It is a series of document templates and checklists that outline important decisions to be made, questions to be answered, and provide a format for recording the vast amounts information each project generates such as user requirements, design decisions, and test plans. - See details.
Lancet uses several types of documents for gathering requirements and documenting project specifications. Each document serves a specific purpose and contains sections and checklists to ensure that appropriate aspects of the task are considered. Since each customers needs are different, the document templates can be modified to meet the distinct needs of each project.
Using standard documentation templates provides many benefits including:

The Lancet methodology has been created to be flexible for projects of many sizes. We want LM3 to be usable, and this means that is can't be overwhelming with too much information. With LM3, you pick and choose the templates and processes that make sense for your project, and ignore the rest.
